Your Key Worker

Each resident at Shenehom Housing Association is matched with a dedicated Key Worker to ensure that guidance and encouragement are always close at hand. Your Key Worker offers support in several crucial areas:

  • Regular One-to-One Meetings: Meet with your Key Worker to discuss your goals, challenges, and achievements, maintaining open communication and a supportive relationship.
  • Care Plan Development and Review: Work together to create an outcome-focused care plan shaped by your own aspirations, with regular reviews to adjust to your progress and changing needs.
  • Emotional Support: Receive personal encouragement and understanding for your mental well-being, helping you feel safe and confident throughout your journey.
  • Practical Support: Get help with everyday tasks like shopping, budgeting, and keeping organised, so daily living becomes more manageable and fulfilling.

  • Financial Guidance: Access support with benefits, budgeting, and managing personal finances, our team helps you build money management skills for independent living.
  • Access to Specialist Services: If needed, we help you connect to GPs, mental health professionals, and a wider network of health and support services.
  • Medication Support: We provide daily living assistance for medication, ensuring safety and promoting independence with a respectful approach.

Advocacy is also a key part of our practical support for mental health housing. You can use your Key Worker as an advocate for your needs, or if you prefer, we can help you access an independent advocate. Our approach guarantees your voice is central in every decision affecting your care and daily life.

At Shenehom Housing Association, practical support is always flexible, designed to empower each resident in a way that suits them best, so achieving independence never feels out of reach.
