Our personalised mental health care plans are designed to reflect strengths, aspirations and changing needs, not just clinical assessments. From the start, we involve residents in every step of the care planning process, believing that collaboration is key to meaningful support and positive long-term mental health outcomes.

Co-created support

Each care plan is developed in partnership with the resident, recognising their right to shape their own path. Residents play an active role, voicing their strengths, needs and personal ambitions.
 

Individual goals

Our goal-led plans include aims for both day-to-day maintenance and positive change. We ask: what brings comfort, what supports recovery, and what new achievements does the resident hope to work towards?

Flexible and responsive

Care plans at Shenehom Housing Association are living documents, evolving as residents grow. Regular six-month reviews ensure continual alignment with changing aspirations.
 

Thorough reviews

Every six months, we hold a complete, collaborative review of each care plan. These reviews are resident-centred and offer a chance to reflect, celebrate progress, and update any goals or support required.

 

Annual clinical input

Each year, a psychiatrist visits (usually at Shenehom) for an in-depth review alongside the resident, staff and, if the resident wishes, family, friends or advocates. This ensures medical and personal perspectives are balanced.

 

Residents' choice and autonomy

Residents are encouraged to invite family members, friends or advocates to take part. We respect privacy and decision-making at each step, ensuring everyone feels valued and heard.
